Waiver and Release
As the accepter/signer of this contract, I agree to abide by all regulations governing the conduct and operation of the instruction sessions, participants, and observers, in order to promote efficiency, safety, and recognition of the instruction certification, as prescribed by the academy. As the accepter/signer of this contract, I understand that there is a risk of personal injury involved in the course of instruction and with this knowledge agree to indemnify and save harmless the academy, instructors, staff, and any other affiliates of the academy from all losses caused by accident or injury to the participants, or to third persons, who may be with the participants of the academy during instructional sessions and/or any other event/activity hosted by the academy. As the acceptor/signer of this contract, I am personally responsible for any damages/losses to the property of the academy, losses/damages to any participants' or third persons' personal property /belongings, or any injuries to participants or third persons' of the instruction session and birthday party. As the accepter/signer of this contract, I agree to be personally responsible for any participant or third person requiring medical attention. The academy, instructors, staff, and/or affiliates of the academy shall not be responsible for damaged, lost or stolen articles, inside or outside the facility. The academy reserves the right to amend or add to these rules and conditions and to adopt new rules and conditions as it may deem necessary. I also agree and understand that all fees paid, including deposits, extra charges, and total party fees/costs are not refundable.
 
For Non-Members:
Please wear comfortable clothing (long, loose, pants, & t-shirt)
If you would like to purchase a Taekwondo uniform of equipment, please check the office.
Parents must be on time when picking up children participants.
